Maxwell-Gumbleton papers: Change of name by deed poll

Know all men by these presents (which are intended to be enrolled in His Majesty's Supreme Court of Jurisdiction) that I the under signed Maxwell Homfray Maxwell- Gumbleton of the Vicarage Chippenham in the County of Wilts Clerk in Holy Orders now Honorary Curate of the Dioceses of Bristol until now or lately called Maxwell Homfray Smith Do hereby on behalf of myself and my heirs and issue lawfully begotten absolutely renounce and abandon the use of my said surname of Smith and in lieu thereof assume and adopt the surname of Maxwell-Gumbleton and for the purpose of widening such change of name I hereby declare that I shall at all times hereafter in all records deeds documents and other writings and in all actions suits and proceedings as well as in all dealings and transactions and matters and things whatsoever and upon all occasions use and subscribe the said name of Maxwell-Gumbleton as my surname in lieu of the said surname of Smith so abandoned as aforesaid. And I therefore hereby expressly authorise and require all persons whomsoever at all times to designate describe and address me and my heirs and issue by such adopted surname of Maxwell-Gumbleton only.

In witness whereof I have hereto subscribed my Christian names of Maxwell Homfray and my adopted and substituted surname of Maxwell-Gumbleton this twenty third day of June One thousand nine hundred and sixteen.

The above named Maxwell Homfray Maxwell-Gumbleton was born of British parents at Kingston in the County of Surrey on the 17th day of June 1872. He has been well known to me from the date of his birth. His father was the father of my mother. The birth certificate now produced and signed on the back by me is the certified copy of an entry in the Register of Births of the above named Maxwell Homfray Smith. [Signature unclear]
